New Feature - Using PatientChase to manage Weight Management enhanced services patients

21st Sep 2021

The enhanced service specification for weight management encourages practices to develop a supportive environment for clinicians to engage with patients living with obesity about their weight.

The specification can be found here:

https://www.england.nhs.uk/publication/enhanced-service-specification-weight-management-2021-22/

In summary, practices are advised to target qualifying patients to refer them to a local weight management service.

Practices can earn £11.50 per patient for doing this.

We have updated PatientChase so that practices can identify, contact and code these patients using our app.

Furthermore, the specification indicates that it is important to differentiate between patients who are BAME and non BAME due to differing BMI thresholds.

This is easy to do in PatientChase.

Please patch up PatientChase to the latest version to use this function (v96 or higher).

How do I know if I'm running the latest version?

At the very top of the main PatientChase screen it should show PatientChase Version xxxxx96 (or higher).

You should also have a green tick near the top right of the screen.

If both of these are not showing, then restart PatientChase to install the latest patch.

A Weight Management PatientChase user guide can be found here.

The most useful and user friendly software that I have encountered in over 30 years of working with management systems.

Cheryl Barton, Data Manager – Bramblys Grange Medical Practice, Hampshire

I am so glad that we as a practice have purchased PatientChase – it has completely revamped our recall system, has been invaluable during flu recall season and more importantly shows us where the ‘holes’ are when we get to near the end of the QoF year. Many thanks!

Jackie Hogg, Clinical Data Manager – The Baffins Surgery, Portsmouth

PatientChase is an excellent product of which I think as soon as you purchase it you will ask yourself ‘how ever did I cope without it?!

Rachel Mardlin, Health Informatics Facilitator – Sheffield University Health

This product will make my QOF staff member’s job so much simpler and is a quarter of a QOF point for me

GP, Newcastle

This is exactly what we’ve been looking for

PM, Cambridge

PatientChase is extremely user friendly and the support provided by PatientChase has been excellent

Practice Administrator, Otley

By using PatientChase, what would normally have taken half a day has just taken 10 minutes!

GP QOF Lead, North Wales

We use PatientChase and find it invaluable, it saves a lot of time and helps us organise our work effectively

Sue Careswell, Leckhampton Surgery

As a practice we have relied upon PatientChase to streamline our patient recall system for several years now. I consider this an essential tool

Updated – Using PatientChase to target patients who need an NHS Health Check

There is now a more integrated way to import NHS Health Check data into PatientChase so it forms part of our Search Builder rather than importing it as a bespoke search.

Important – Are you using Advanced Searches or importing other extra searches into PatientChase at the point of synchronising with EMIS? If so, please read this update.

Many practices load extra data into PatientChase such as ‘review done’ data, medication review data, weight management data etc. If you are familiar with this, then you will need to take action. This only applies if you export searches from EMIS prior to syncing PatientChase with EMIS. If you simply ‘EMIS Sync’ PatientChase with EMIS when you open it, then you do not need to take action and please continue to use PatientChase as normal.
